Permission Slips

If you have a young person that will be participating in youth activities that might occur off church premises we will need to have a completed permission slip on file.  Here is the Permission Slip Form.

Background Checks

Our children and youth are the most precious people that God has entrusted us with. We take every step possible to guard and protect them.  We do background checks on all of our staff and volunteers that participate in ministering to them.  If you will be working with our young people here at Bethel, please fill out and return the following forms to the pastor or staff member for processing. Background Check Authorization and Background Check Application

Children Safety Policy

It is important for anyone working with our children to understand and abide by our children safety policy.  If you will be working with our children, please read and fill out the children safety policy form and return it to the pastor or a staff member. Children Safety Policy
